Position Summary:
Unison Health is seeking a Case Manager to join our Home-Based Treatment (IHBT) team, providing services to youth and families with complex behavioral health needs. This role is ideal for professionals who value flexible, community-based work, small caseloads, and the opportunity to make a long-term impact through short-term, intensive support.
Key Responsibilities & Role Highlights:
- Serve a limited number of youth and families (small caseload)
- Flexible, community-based hours – appointments take place in the home or community
- Team-based, collaborative work environment
- Extensive training and professional development opportunities
- Support for career growth, including supervision for those pursuing licensure
- Participation in 24/7 on-call rotation, with premium call pay included
- Deliver intensive home-based case management services in accordance with state mandates and agency policies
- Collaborate with clinical therapists to support individualized family service plans
- Document all services provided and maintain accurate client records in the electronic health record system
- Ensure compliance with confidentiality guidelines and proper handling of release of information
Education & Experience Requirements:
- Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field
- Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
- Valid driver’s license and reliable transportation required
- Must be insurable under the agency’s commercial liability policy
- Experience working with children and families preferred
- Must demonstrate knowledge in:
- Mental illness and treatment
- Individual and group counseling
- Behavioral health assessments and crisis intervention
