OverviewSentara Regional Resource Pool located in Hampton Roads is hiring an Inpatient Case Manager, MSW (Days/PRN for the Resource Pool). The role requires working at four hospitals in either the Southside or Western Tidewater regions. Location and unit assignment are based on greatest need within the chosen region. Candidates must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
Location& Region Options
- Southside only (Norfolk General, Leigh, Princess Anne, VA Beach General)
- Peninsula only (Obici, Careplex, Williamsburg Regional, Albemarle Medical Center)
- Western Tidewater (combination of 2 Southside and 2 Peninsula: SNGH, SLH, SPAH, SVBGH, SOH, SCH, SWRMC, SAMC)
- Operational hours: 8:00 AM – 4:30 PM, 7 days per week.

Hours:
Must self‑schedule at least 48 hours of prescheduled time per calendar month, including at least 16 hours of weekend availability.
Master’s degree in Social Work
Certification / Licensure- Certification in Case Management (CCM, CMAC, or ACM‑SW) required within one year of eligibility.
- BLS required within 90 days of hire.
- One year of related experience.
- Must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
As an Inpatient Care Manager, you will provide medical social work services to patients receiving care in the hospital setting.
Responsibilities include:
- Assessing the social, psychological, cultural, environmental, and financial situations of each patient.
- Collaborating with patients, families, healthcare team members, and community organizations.
- Using advocacy skills to identify needs and resources within the hospital and community.
- Developing and evaluating effective transition plans based on diagnosis, prognosis, course of treatment, past services, short‑term and long‑term goals, provider options, and available health‑care benefits.
