Position Summary
Quality Health Care Group Inc. is seeking a Licensed Registered Dietitian (RD) to provide nutrition assessment, counseling, and care planning to patients in a home health care setting. The RD will work under the supervision of a Registered Nurse as part of a multidisciplinary care team, supporting patients in achieving optimal health outcomes through evidence-based nutritional therapy. This is a 1099 independent contractor position, offering flexible scheduling and patient-focused work.
Key Responsibilities- Conduct comprehensive nutrition assessments for patients referred through home health services.
- Develop, implement, and evaluate individualized nutrition care plans in collaboration with the supervising RN.
- Provide Medical Nutrition Therapy (MNT) for patients with chronic conditions such as diabetes, hypertension, renal disease, gastrointestinal disorders, obesity, and other nutrition-related diagnoses.
- Educate patients and caregivers on dietary modifications, meal planning, and healthy lifestyle practices to support medical treatment plans.
- Monitor patient progress, document care provided, and update care plans as needed in compliance with home health regulations.
- Collaborate with physicians, nurses, therapists, and other members of the care team to ensure integrated patient-centered care.
- Maintain accurate and timely clinical records in accordance with HIPAA and agency requirements.
- Stay current with professional standards, continuing education, and regulatory changes related to home health and nutrition care.
- Current Registered Dietitian (RD/RDN) license in the state of Pennsylvania.
- Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field from an accredited program.
- At least 1 year of experience in clinical or community nutrition; prior home health care experience preferred.
- Familiarity with Medicaid/Medicare coverage for MNT and compliance with CMS and state regulations.
- Strong communication and patient education skills with the ability to work independently.
- Must maintain professional liability insurance as a 1099 contractor.
- Reliable transportation to conduct in‑home visits (if required).
- Contract Basis (1099):
Flexible schedule, per‑visit or hourly compensation model. - Contractor is responsible for own taxes, insurance, and licensing fees.
- Company will provide referrals, scheduling coordination, and EMR documentation access.
