Psych Nurse Practitioner (Outpatient) Community Health –South Shore

Great opportunity for a psychiatric nurse practitioner to join a growing team working in a community health environment providing care to the under-served population.

The practitioner will work with a team of psychiatrist, Primary Care Physicians and Social Workers to provide comprehensive and appropriate continuum of care.

Duties include performing psychiatric diagnostic assessments for all referred patients. Prescribe medications and provide medication management.

Assess, plan, document and evaluate patient needs and treatment.

Provide consultation for primary care providers regarding patient treatment.

Provide intervention and referral for patients when appropriate.

Provide counseling, crisis intervention and advocacy to all patients and their families, taking into account their unique cultural backgrounds.

Psycho education around such issues as high-risk behavior, mental health, addictions, and domestic violence is also a major component of the job duties.
