EBCAP serves as the backbone organization for Health Equity Zones across East Providence, Bristol, and Warren—and we're looking for a passionate community connector to join our Family Development team. This isn't a desk job. This is about showing up where people are: at parks, food pantries, libraries, community centers, and local gathering spaces, building trust one conversation at a time.
What You'll DoMeet People Where They Are (50% of your time)
- Take EBCAP and Health Equity Zone services directly into the community—setting up at farmers markets, schools, senior centers, local businesses, and anywhere people naturally gather
- Build genuine relationships with individuals and families, becoming a trusted advocate who helps break down barriers to health and wellbeing
- Create warm handoffs to services and resources, walking alongside community members until they feel confident navigating systems independently
- Document your impact through our client database to help us prove what works
Connect the Dots (35% of your time)
- Collaborate across healthcare, behavioral health, and social services to create seamless pathways to care
- Participate in strategic meetings—from our HEZ Steering Committee to the Opioid Settlement Task Force—where your community insights will shape program decisions
- Stay current on resource changes so you're always equipped to guide people to the right support at the right time
- Combat social isolation by linking people to health services, support groups, and community connections
Drive Innovation (15% of your time)
- Support grant-funded initiatives by developing educational materials, facilitating community programs, and planning events
- Build a comprehensive resource inventory that becomes the go-to guide for our entire team
- Jump in to support EBCAP programs like our food pantry when extra hands mean better service
- Bring your ideas to the table—we're building something special, and your voice matters
You bring:
- A high school diploma or equivalent (Associate degree in social science, public health, or related field is a plus)
- At least 1 year of community engagement or outreach experience
- Comfort with Microsoft Office (Excel, Word)
- Community Health Worker certification through the RI Certification Board OR the ability to obtain it within 12 months
You have:
- Deep roots or strong connections in East Providence, Bristol, or Warren communities
- Natural warmth that makes people feel comfortable asking for help
- The ability to work independently out in the community while staying connected to your team
- Strong organizational skills and attention to follow-through
- Physical stamina to be on your feet, navigate community spaces, and lift up to 25 pounds
- Reliable transportation to bring materials and resources where they're needed
- Bilingual/bicultural representing the populations we serve (Spanish, Cabo Verdean Creole, Haitian Creole)
- Lived experience navigating health or social service systems
- Existing relationships within local communities
