Health Coach Specialist II – Denver, CO (Field-Based)

Program &Position Overview
Freedom Care of Colorado specializes in Medicaid home‑care, hiring trusted family members or aides to deliver patient‑centered care. The Health Coach Specialist II works with waiver members, caregivers and stakeholders to ensure smooth, compliant service and optimal use of authorized hours.
This role is field‑based; you may also work from home or the Colorado office up to 50% of the time when not in the field.
Every Day You Will- Schedule and perform quarterly supervisory visits for Home Care Compliance.
- Act as a field expert, supporting members and caregivers in continuing care and optimal utilization of authorized hours.
- Conduct home inspections to ensure safety and compliance of participant’s home and surroundings.
- Partner with the care team to review daily notes for signs that extra training or support may be needed, documenting all contact in Salesforce.
- Maintain contact with caregivers—virtual or in‑person—based on needs in the service plan.
- Report concerns about member or caregiver health, safety or unmet needs to the staff RN or Case Manager.
- Track all activities via CRM (Salesforce).
- Operate efficiently within a fast‑paced startup culture.
- Perform other duties as required or assigned.
- Bachelor’s degree or equivalent work experience.
- Two or more years of bedside experience (CNA, QMAP, Direct Care Worker, Medical Assistant, etc.).
- Minimum one year of call‑center, telephonic outreach or customer‑service experience (required).
- Independently exercise judgment and voice concerns or trends.
- Strong verbal and written communication skills with the ability to establish rapport.
- Strong problem‑solving skills and ability to recognize and escalak issues when appropriate.
- Excellent time‑management, critical‑thinking and active‑listening; able to multitask.
- High empathy and emotional intelligence.
- Computer proficiency, Salesforce experience a plus.
- Comfortable in a dynamic, constantly evolving startup environment.
- Maintain confidentiality and adhere to HIPAA guidelines.

CompensationRange
$23–$26 USD per hour.
