The Alzheimer Society of Thunder Bay is looking for a motivated, experienced, and driven individual to join their team!

Reporting to the Executive Director, the First Link Care Navigator will coordinate and integrate supports and services around the person living with dementia and their care partner. In this direct client service role, they will be the first-point-of-contact across the region for families given a dementia diagnosis. The successful candidate will be responsible for identifying needs, supporting self management goals, and strengthening the communication and care planning link between providers and across sectors along the continuum of care.

Key Responsibilities
  • Pro-actively manage incoming First Link referrals to facilitate early care navigation
    support
  • Gather information and conduct or review assessments with clients to identify their
    current and future needs and goals
  • Coordinate care across internal and external service providers, and advocate and
    support clients in navigating these resources and systems
  • Establish intervention plans to meet client needs using a person/family-centred approach
  • Facilitate regular and ongoing care conferences between clients and their care teams
  • Develop strategies to address/overcome barriers in effective coordination/integration of internal and external supports and services
  • Support awareness of First Link to health professionals, service providers and relevant community stakeholders in collaboration with internal and external partners
  • Plan, implement, and evaluate First Link referral, intake, navigation, care coordination and proactive follow-up functions to ensure a timely response to emerging needs
  • Collect, maintain and report required quantitative and qualitative data to support
    province-wide monitoring, evaluation and reporting
  • Keep informed about Alzheimer’s disease and other dementias, clinical manifestations, behaviours, current care practices, treatment options, placement options, community resources, and relevant legislation
  • Participate in knowledge transfer and exchange and collaborate with Alzheimer Societies across Ontario to support the delivery of best practices and ongoing quality improvement
  • Perform other duties as required
Location

This position is a home-office position, with frequent travel expected to serve the following communities, including but not limited to:
Nipigon, Schrieber, Terrace Bay, Marathon, Manitouwadge, Geraldton, Longlac and the surrounding First Nation communities. Preference will be given to candidates residing in this area. Travel is expected to be around 75%, with mileage and commute time compensated.

Compensation

CA $60,000.00 per year

Qualifications
  • Bachelor’s degree in social work, nursing, gerontology or related health care discipline
  • Master’s degree would be considered an asset
  • 3-5 years experience providing client service in the health and/or social service sectors
  • Experience working with people living with Alzheimer’s disease or other dementias an asset
  • Experience in management of chronic and complex health conditions
  • Experience building partnerships and collaborating with external organizations
  • Experience in assessment and care planning/coordination
  • Experience facilitating educational sessions and/or support groups
  • Knowledge of community services/supports, primary care, specialized geriatric services, and clinical, social and residential care options
  • Knowledge of clinical practices and training models related to dementia (e.g.:
    P.I.E.C.E.S. and U-First!)
  • Advanced verbal and written communication skills
  • Fluency in French or another language considered an asset
  • Proficiency with Microsoft office, case management software, a care coordination systems
Requirements
  • Valid G driver’s licence and reliable transportation
  • Must be registered in good standing with a regulated health care body
  • Must prove professional registration and carry $2 million professional liability coverage
