Mental Health Team – Adult Social Care and Health
We are seeking dedicated and motivated individuals to join our Mental Health Team as Assistant Social Work Practitioners. Both full‑time and part‑time positions are available.
Key Responsibilities- Conduct comprehensive assessments and reviews of service users and carers, ensuring compliance with statutory requirements.
- Identify risks, needs, and appropriate interventions, promoting independence through a strengths‑based approach.
- Develop and implement innovative care packages, including preventative work and risk assessments, utilizing community resources such as reablement and telehealthcare.
- Maintain up‑to‑date knowledge of relevant legislation, including the Mental Health Act, Mental Capacity Act, Deprivation of Liberty Safeguards, Care Act (with Making Safeguarding Personal), and the Human Rights Act.
- Collaborate effectively with individuals, families, carers, and professionals across health, housing, and partner organisations.
- Demonstrable understanding of the legislative frameworks underpinning adult social care and mental health practice.
- Strong assessment, planning, and communication skills.
- Commitment to personal and professional development, with opportunities for further training and skill enhancement.
