We're expanding our home-based services to help more families access the support they need, creating an exciting opportunity for someone passionate about participant empowerment to shape how we deliver individualized services in community settings.
About This OpportunityAs our Self-Directed Assistant for Home-Based Services, you'll work directly with participants and families to help them manage their funding and direct their own services. Based in Libertyville with 50% travel to participant homes throughout the area, you'll provide the guidance and support that enables families to make informed choices about the services that matter most to them.
What You'll Accomplish- Design individualized budgets that help families maximize their Home-Based Services funding
- Build strong partnerships with participants and families through regular home visits and responsive support
- Create documentation systems that ensure compliance while respecting participant autonomy and choice
- Connect families to valuable community resources that expand access to meaningful support
You bring experience supporting adults with developmental disabilities and a collaborative approach to helping families navigate complex systems. Your ability to explain financial processes clearly and your commitment to participant self-determination make you well-suited for this role.
- Bachelor's degree in Special Education or related human service field
- Minimum one year experience working with adults with intellectual and developmental disabilities
- QIDP certification or commitment to complete training within six months of hire
- Valid driver's license from Indiana, Illinois, or Wisconsin with reliable transportation
- Proficiency with budgeting software, documentation systems, and standard office technology
Work Environment: Beautiful 72-acre campus in Libertyville with regular travel to participant homes throughout the area
