OverviewThe Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. It provides 89 beds of skilled nursing care, including a 18‑bed Special Care Unit for residents with Alzheimer’s disease or related conditions. The facility serves veterans, their spouses, and eligible “Gold Star” parents.
PrimaryJob Duties
- Perform basic nursing care: bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, and social interaction to meet residents’ basic needs.
- Assist nursing staff with resident safety, mobility, and use of assistive devices; perform CPR as needed.
- Maintain safety of residents through correct lifting techniques, environmental monitoring, use of safety devices, and monitoring resident whereabouts.
- Conduct basic resident assessments including weights, bowel and bladder incontinence, intake and output, vital signs, and charting.
- Implement infection control practices via standard precautions.
- Maintain equipment: check and maintain equipment, replace mattresses, foot cradles, lift batteries, clean oxygen concentrators, fill portable tanks, disinfect tubs and showers.
- Transport residents for medical appointments and attend required training and staff meetings.
- Provide inter‑shift communication and participate in restorative or GAPA meetings.
- High school diploma or GED.
- At least one (1) year of professional experience as a CNA or equivalent duties.
- A valid Nurse Aide Certification (CNA) from the Colorado Department of Regulatory Agencies is required.
- A valid practical nursing license (LPN) from Colorado or a state participating in the Nurse Licensure Compact can substitute for CNA certification.
- Current and active RN license renewal required.
- Full background check required.
- Willingness to travel to conferences and training sessions.
- Valid driver’s license required.
- Must be willing to take a TB test.
- Shift work: day, evening, night, weekends, and holidays as needed.
- On‑call rotation as defined in facility protocol.
