What You’ll Be Doing
  • Working primarily remotely, Health guides will collaborate with the patient’s medical practice staff and Guidehealth’s Case Management team to manage a targeted population of patients.
  • Functioning independently while being a highly collaborative team member as an extension of the medical practice.
  • Assist patients in achieving better health outcomes by addressing questions and concerns, connecting them with the medical practice, supporting high-impact care plans, closing care gaps, escalating medical issues, and guiding referrals to specialty care and community-based organizations (CBOs).
  • Accurately document patients’ medical history and issues in the electronic health record (EHR).
  • Understand the referral and prior authorization process.
  • Build and maintain a trusting, professional working relationship with assigned medical practices and within the Guidehealth organization.
  • Implement and meet assigned Key Performance Indicators to produce positive patient health outcomes while decreasing medical spend.
  • Use effective communication modalities (calls, texts, portals, emails) to engage and schedule high-risk patients.
  • Escalate medical issues appropriately to the Health Guide Registered Nurse, Primary Care Physician, medical practice, or Guidehealth leadership.
  • Follow all security policies to protect PHI under Guidehealth’s custodianship and Guidehealth Intellectual Property.
  • Position may require travel (10-15%) to clinical sites in the Atlanta, GA area for patient and provider engagement and retinal eye exams, with paid training provided.
Qualifications
  • Certified or Registered Medical Assistant with 2+ years of experience.
  • Must be bilingual (English/Spanish).
  • A love for patient coordination to serve people with complex medical and social needs.
  • Willingness to become proficient in motivational interviewing, critical listening, and behavioral change management to improve patient engagement.
  • Strong interpersonal skills with empathy, patience, professionalism, and respect in all interactions.
  • Exceptional ability to document patient interviews accurately using software and in real time.
  • Experience with medical terminology and the function of a primary care physician practice.
  • Knowledge of social determinants of health.
  • Ability to navigate technology across multiple platforms (EHRs, care management analytics databases, phone dialing system, Microsoft Office).
  • Proficiency in Microsoft Office:
    Outlook, Word, Excel.
  • Familiarity with EHR systems (eClinical

    Works, EPIC, Athena, Cerner) and ability to balance priorities with attention to detail.
  • Excellent written, visual, and verbal communication skills.
  • Ability to track multiple tasks and ensure completion.
  • Ability to work independently as part of a patient’s care team under supervision of licensed clinical professionals.
  • Demonstrated ability to work effectively in a remote work environment.
  • Reliable transportation and a valid state driver’s license for occasional travel to provider sites.
  • Adherence to patient confidentiality when using a headset in a private home office.
  • Compliance with all Guidehealth policies and procedures.
