Community Health Worker Mobile Health Unit

Job Title: Community Health Worker I & II

Job Status: Full-time

Job Summary: Under general supervision the Community Health Worker I & II will conduct wellness consultations that include Social Determinants of Health screenings, health coaching, health education, and referrals to local community services. Promote preventative care, health screenings, and collaborate with health care providers to enhance community outreach.

Essential Duties And Responsibilities
  • Conduct SDOH screenings to identify individual or family needs.
  • Refer clients to community resources and agencies to meet identified needs.
  • Outreach in the community to promote, maintain, and improve health.
  • Assist clients in navigating and accessing services and adopting healthy behaviors.
  • Provide health education for chronic disease prevention (e.g., blood pressure, cholesterol, diabetes).
  • Perform health screenings such as vital sign intake, HIV testing, lead testing, and point‑of‑care testing.
  • Document details of client engagement.
  • Follow‑up with clients to determine if needs are met or additional support is required.
  • Assess client eligibility for services such as financial assistance, insurance, and other programs.
  • Help clients obtain health care services (Medicaid, family planning, connections to medical homes, etc.).
  • Act as liaison between communities, individuals, and service providers.
  • Recruit new clients and strengthen referral partnerships.
  • Participate in continuing education and attend community meetings and health fairs.
  • Operate office equipment and software applications as required.
  • Perform other duties as assigned.
Knowledge, Skills, and Abilities
  • Identify impacts of Social Determinants of Health on overall well‑being.
  • Understand case management and care coordination.
  • Know local resources and agency programs for health promotion.
  • Experience in community outreach, health promotion/education, or public health.
  • Provide health coaching and education.
  • Operate standard office equipment and use requisite software (Microsoft Office).
  • Maintain confidentiality and follow HIPAA standards.
  • Work independently, problem‑solve, prioritize, and adapt to complex situations.
  • Demonstrate strong oral and written communication and the ability to build rapport with diverse populations.
Educational and Experience Requirements
  • Minimum:
    High school diploma or GED equivalent.
  • Level I – Entry level, 6 months to 2 years experience as a Community Health Worker.
  • Level II – At least 2 years’ experience as a Community Health Worker.
Licenses and Certifications
  • Current CHW certification preferred or willingness to complete during onboarding.
  • Valid driver’s license and dependable transportation.
  • Level I must complete 12‑month apprenticeship program once CHWA certified.
  • Level II must hold MiCHWA training certificate.
Working Conditions
  • Hours:

    Normal business hours; additional hours may be required.
  • Travel:
    Local, in‑state travel, up to 90%.
  • Environment:
    Mobile unit, climate‑controlled office, and/or remote work.
