Complex Case Coordination (C3) Liaison
Wounded Warrior Project (WWP) seeks a dedicated professional to respond to crisis referrals of warriors, perform initial assessments, guide warriors towards appropriate internal or external resources, act as a case manager for treatment planning, and eliminate barriers to care as appropriate in a mission‑focused, collaborative team environment.

- Serve as subject‑matter expert and resource in planning, coordination, and execution of mental health programs.
- Maintain caseload of assigned warriors.
- Perform comprehensive screenings and needs assessments to identify and coordinate appropriate external care.
- Partner closely with warriors’ care teams to develop and implement treatment plans aligned with the warrior’s goals.
- Develop collaborative relationships to maximize informed decision‑making.
- Collect and maintain sensitive health information according to standards.
- Facilitate admission and discharge planning with internal and external resources.
- Coordinate with families and advocates to ensure post‑discharge services.
- Maintain accurate and timely case documentation.
- Provide transition support with programmatic referrals to WWP programs.
- Coordinate comprehensive support and care through internal programs and external organizations.
- Collect documentation to facilitate payment for WWP‑funded care.
- Input data into WWP systems ensuring accurate updates.
- Empower warriors by mitigating crisis challenges and removing barriers to care.
- Develop understanding of treatment options and external resources.
- Provide feedback to program leadership for process improvement.
- Identify and elevate issues impacting Warrior care to leadership.
- Cultivate strong partnerships with internal programs, external agencies, and veteran service organizations.
- Work with DoD, VBA, VHA to coordinate professional services and care.
- Prepare briefings, reports, and status updates.
- Attend required training and meetings.
- Other related duties as assigned.
- Strong analytical and problem‑solving skills.
- Strong understanding of mental health conditions, treatment modalities, and facility types.
- Attention to detail and accuracy.
- Ability to adapt to evolving procedures, priorities, and deadlines.
- Stakeholder interaction and information gathering abilities.
- Effective communication with diverse personalities and sensitive matters.
- Excellent written and verbal communication and documentation.
- Collaboration within team, problem identification and solution.
- Process improvement recommendation.
- Familiarity with community and government mental health resources.
- Knowledge of military and veteran issues and impact.
- Relationship building.
- Understanding of Wounded Warrior Project mission and services.
- Understanding of VA programs and Community Care Network providers.
- Strong organizational and time‑management skills.
- Proficiency with Microsoft Office and Teams.

- Four years of experience as a social worker or in a mental‑health related field.
- Four years experience working with veteran healthcare‑focused programs and case management.
- Four years experience working with military and veteran population in support of development and readjustment.
- Five years experience working with local community and government resources for warriors in crisis.
- Five years experience supporting wounded service members with visible/invisible injuries.
- Five years experience as a social worker.
- Proficiency in Salesforce or other CRM system.
- Bachelor’s degree in social work, psychology, rehabilitation counseling, case management, or related mental health field.
- Equivalent combination of education, training, certification, and experience may be considered.
