Role DescriptionAs a Health guide, you will be supported by clinical teams in strengthening the connection between the patient and their primary care physician/medical practice staff, improving the patient’s health and well‑being. You will engage patients, help them navigate care, solve healthcare issues, and improve communication with their medical practice.
What You’ll Be Doing- Working primarily remotely, collaborating with the patient’s medical practice staff and Guidehealth’s Case Management team to manage a targeted population of patients.
- Functioning independently while being a highly collaborative team member as an extension of medical practice.
- Assisting patients in achieving better health outcomes by addressing questions and concerns, connecting them with the medical practice, partnering on high‑impact care plans, closing care gaps, escalating medical issues, and navigating them to the right resources, including referrals to specialty care and community‑based organizations.
- Accurately documenting patients’ medical history and issues in the electronic health record (EHR).
- Understanding the referral and prior authorisation process.
- Continuously building a trusting, professional relationship with assigned medical practices and within the Guidehealth organization.
- Implementing and meeting assigned Key Performance Indicators to produce positive patient health outcomes while decreasing medical spend.
- Using effective communication modalities (calls, texts, portals, emails) to engage and schedule high‑risk patients.
- Appropriately escalating medical issues to the Health Guide Registered Nurse, Primary Care Physician, medical practice or Guidehealth leadership.
- Following all security policies and procedures to protect PHI and PII under Guidehealth’s custodianship.
- Position may require compensated travel as needed (10‑15%) to clinical sites in the Atlanta, GA area for patient and provider engagement and conducting retinal eye exams with paid training provided.
- Certified Registered Medical Assistant with 2+ years of experience.
- Must be bilingual (English/Spanish).
- A love for patient coordination to serve people with complex medical and social needs.
- Willingness to become proficient in motivational interviewing, critical listening skills, and behavioural change management.
- Strong interpersonal skills with a focus on empathy, patience, professionalism, and respect.
- Exceptional conversation skills and the ability to document patient interviews precisely.
- Experience and understanding of medical terminology and primary care practice functions.
- Knowledge of social determinants of health.
- Competency in navigating technology across multiple platforms (EHR, care‑management analytics, dialing system, Microsoft Office).
- Proficiency in Microsoft Office (Outlook, Word, Excel).
- Familiarity with EHR systems such as eClinical
Works, EPIC, Athena, Cerner. - Exceptional written, visual and verbal communication skills.
- Ability to track progress of multiple tasks and ensure completion.
- Ability to work independently as part of a patient’s overall care team under licensed clinical supervision.
- Successful remote work experience.
- Reliable transportation and a valid state driver’s licence for occasional travel.
- Adherence to patient confidentiality using a private headset during all conversations.
- Performance of other duties as assigned.
- Compliance with all Guidehealth policies and procedures.
