Job Summary: The Universal Health Care Worker combines the roles of Personal Care Assistant, Medication Technician, and Home Health Aide allowing the worker to assist both in a communal living environment or home based services. The Universal Health Care Worker is required to complete all education, training, and certification required to perform all assigned tasks.

Essential Function / Core Competencies

  • Assist with Activities of Daily Living (ADLs) and Personal Care:
    Provide support with personal hygiene, bathing, grooming, dressing, oral care, and elimination needs. Encourage and support residents to maintain the highest level of independence and use supportive devices when needed. Ensure comfort and cleanliness, including proper hydration and nutritional intake. All personal care provided should be documented at the time of service in the medical record.
  • Provide Companionship and Emotional Support:
    Engage with residents to provide companionship, encourage participation in activities and social functions, and support emotional well-being. Assist by accompanying to events or assist with scheduling transportation services as needed.
  • Medication Assistance and Documentation (medication technician only):
    Assist residents with the self-administration of medications in accordance with the five medication rights (right resident, medication, dose, time, and route). Document medication administration, care plans, and other services provided in electronic medical records (EMR).
  • Ensure Resident Safety and Comfort:
    Make regular rounds to ensure residents are comfortable, safe, and in no distress. Report changes in condition to the nurse supervisor immediately and assist with any required interventions.
  • Maintain a Safe and Clean Environment:
    Keep the resident's living area, common areas, and equipment clean, safe, and free from hazards. Follow infection control guidelines, ensuring proper sanitation and hygiene, including the use of gloves and other safety measures.
  • Provide Nutritional Support and Assistance:
    Assist with meal preparation, serving meals, and ensuring that residents receive adequate nourishment. Adhere to any physician prescribed diets. Monitor intake and output as directed, and maintain a constant supply of fresh water and extra nourishment when allowed.
  • Assist with Mobility and Transportation:
    Encourage and assist residents with mobility, whether it be walking, using assistive devices, or during transportation for appointments. Provide support for transportation when needed, including scheduling transportation and accompanying residents to outings.
  • Support in Admissions, Transfers, and Discharges:
    Assist with the safe admission, transfer, or discharge of residents. Ensure that personal items, clothing, and equipment are properly stored and labeled according to facility/agency policies.
  • Participate in Care Planning and Communication:
    Attend care conferences, in-service training, and team meetings. Contribute input on case progress, care plans, and any changes to the resident's condition. Report any discrepancies in care plans or concerns to the licensed nurse.
  • Provide Pet and Housekeeping Support:
    In certain cases, assist with pet care (walking, feeding, cleaning), as well as other light housekeeping tasks, such as cleaning and maintaining the living environment to ensure a comfortable and safe setting for the resident.

Must maintain a professional, respectful, friendly demeanor with all residents, family members, guests, and staff at all times.
