Position Summary

The Care Coordinator position provides case management for an assigned caseload of clients under People Care Health Services. The primary goal of case management is to ensure the delivery of appropriate services based on the client’s care plan and that services are being provided properly and with quality by assigned care staff. In addition, case management includes coordination of care with clients, families, medical personnel, and any other agencies providing services.

Responsibilities
  • Maintain assigned evolving caseload.
  • In coordination with Agency Manager and program team members, schedule, complete, prep, and document state-required 90‑day visits and/or continued stay reviews (CSRs).
  • Prep and perform intake assessment of new clients for HCBS, VA, Region 10, and Private Pay.
  • Select, schedule, and coordinate caregivers based on assessment information and care plan information for new and existing clients/consumers.
  • Attend meetings as assigned.
  • Assist schedulers when responding to scheduling conflicts, emergencies, and/or “call outs.” Client schedule must be complete for the next business day/weekend before leaving each day. Work client shifts as required to meet regulatory requirements and company policy.
  • Contact clients/employees via telephone or drop‑in visit, on a scheduled or impromptu basis to receive feedback and ensure service meets the goals outlined in the care plan. Document and respond promptly to complaints, and review complaints with the Agency Manager.
  • Ensure appropriate paperwork on an annual basis for any eligible employee is submitted in a timely manner (EVV).
  • Participate and support annual training as required.
  • Keep all client files current and in compliance.
  • Accept on‑call duty as assigned by the Agency Manager.
  • Provide onboarding caregiver check‑off as needed.
  • Enter necessary quality management and reporting responsibilities and reports as assigned by the Agency Manager.
  • Maintain knowledge of and ensure company compliance with all applicable local, state, and federal laws and regulations concerning the operation and provision of home care service personnel, including continuing professional education requirements.
  • Support recruitment, hiring, and training of caregivers, including protocols and processes.
  • Be available for all survey and audit requirements and assignments.
  • Complete monthly self‑performance journal.
Qualifications
  • High school diploma or equivalent required.
  • Licensure/Certification/Registration:
    None.
  • Experience in the field of home health care preferred but not required.
  • Experience managing a home and family or caring for disabled individuals highly desirable.
  • Professional experience in customer service.
  • Special qualifications:
    None.

Working Conditions
  • Prolonged periods of sitting at a desk and working on a computer.
  • Essential physical requirements include standing, kneeling, squatting, stooping, or typing.
  • Physical effort/lifting up to 50 pounds.
  • May be required to work weekends, nights, or be on‑call regularly.

Travel Requirements

Travel up to 50% of the time. Employees must follow all travel & mileage guidelines.
