Overview
Clinical Pharmacist - Remote Services reports to the Pharmacist-in-Charge and provides remote medication interpretation, evaluation, preparation, and dispensing for Brown University Health affiliates. This role collaborates with multidisciplinary care providers to ensure safe, appropriate, and cost-effective medication use.
Responsibilities- Receive patient medication orders remotely and review and interpret each order for therapeutic appropriateness and patient safety.
- Apply pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective medication use for patient populations served.
- Provide drug information and consultation to prescribers, nurses, and other health care professionals.
- Proactively review and approve medication orders in the electronic health record (EHR).
- Identify, document, resolve, monitor, and prevent drug therapy–related problems.
- Engage with other clinical pharmacists or clinical pharmacist specialists for complex clinical situations.
- Integrate patient-focused medication therapy management and pharmaceutical care according to established pathways or guidelines.
- Participate in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications and recommend dose adjustments and laboratory monitoring as required.
- Ensure compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs.
- Document and report all clinical interventions, medication errors, near misses, and adverse drug events per protocols.
- Remotely review and approve formulations for sterile products requiring compounding or manipulation prior to dispensing and complete in-process and final checks for medications prepared by pharmacy technicians.
- Participate in continuous quality improvement activities and total quality management initiatives, including adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs.
- Maintain professional competence through participation in pharmacy staff development and skills enhancement programs.
- Perform other duties as assigned.
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
- Current unrestricted licensure as a licensed pharmacist in Rhode Island and as a registered pharmacist in Massachusetts.
- Within 90 days of start date, possess and maintain current unrestricted licensure as a licensed pharmacist in Rhode Island.
- Within 18 months of hire, possess and maintain Board of Pharmacy Specialties certification in clinical pharmacy expertise (e.g., Pharmacotherapy (BCPS), Oncology (BCOP), Cardiology (BCCP)).
- Successful completion of Post Graduate Year 1 (PGY‑1) Residency from an accredited program and/or a minimum of 3 years clinical experience.
- Effective interpersonal and communication skills, both written and verbal, to interact appropriately with colleagues and others.
- Effective organizational and presentation skills.
- Knowledge of specialty technology and experience with MS Word, Excel, Outlook, and web browser.
Physical Requirements
Employee will establish a secure remote workspace to protect confidential and protected health information (PHI) and protected identifiable information (PII). Employee will be responsible for costs associated with setting up, furnishing, and managing the remote workspace and maintaining internet connectivity. Requires visual and manual dexterity to operate a computer. Occasional travel to Brown Health affiliates may be required.
Pay Range$50.49–$83.30 per day per diem.
