Per Diem Medical Social Worker (MSW) - Home Health

The Per Diem MSW works collaboratively with our interdisciplinary care team to address the social, emotional, and environmental needs of patients receiving skilled home health services. This role offers flexibility, autonomy, and meaningful patient impact.

• Conduct psychosocial assessments for home health patients
• Provide counseling, emotional support, and crisis intervention as needed
• Assist patients and families with community resources, financial assistance, advance directives, and care planning
• Collaborate with nurses, therapists, physicians, and office staff to support safe care in the home
• Participate in care coordination and discharge planning
• Complete accurate, timely documentation in the electronic medical record
• Educate patients and families on available services and support systems

• Master’s degree in Social Work (MSW) from an accredited program
• Current Connecticut Social Work license (LMSW or LCSW)
• Home health or healthcare experience preferred (but not required)
• Strong communication, organization, and time‑management skills
• Ability to work independently in the community
• Valid driver’s license and reliable transportation
