** Position Status:

* Hours:

** Mon & Tue 8:00-5:00pm, Wed & Thurs 8:00-6:00pm, Fri 8:00-2:00pm,2nd & 4th Saturdays 9:00-12:00 pm
** Night or weekend flexibility is needed for occasional community events.
**** Overview
* * Works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
** Education
* ** High School Diploma or Equivalent required
** Certification/Licensure
*** Driver’s License-Certification-Other/National
** Experience
* ** Experience working in a community-based setting preferred
* Experience working in community events and organizing events is preferred.
* Knowledge of medical terminology is preferred.
