Our team provides an island-wide assessment and care planning service for adults with learning disabilities. You’ll play a key role in supporting individuals aged 18-65, ensuring they receive the right care and support to live fulfilling lives.
Mainduties of the job
In this role, you will organise and facilitate assessments, develop and implement personalised care plans, and apply social work ethical principles and values in practice. You’ll work within the FACs Operational Guidance to maintain high standards and consistent processes. Your expertise and commitment will help deliver a service that makes a real impact. This role also qualifies for a recruitment incentive of £3,000, payable in the 1st, 13th, and 25th months of employment.

- Social Work Professional Qualification (e.g. BA/MA Post Graduate Diploma in Social Work, Dip. SW, CQSW).
