Role Description

This is a part-time Physical Therapist role based in Brenham, TX, serving patients in their homes. The Physical Therapist is responsible for evaluating patient functional status, developing individualized treatment plans, and providing skilled therapeutic interventions to improve mobility, reduce pain, and restore functional independence. The role requires collaboration with patients, family members, physicians, and interdisciplinary care team members to ensure coordinated and effective care.

The Physical Therapist will also provide patient and caregiver education, monitor progress, and adjust treatment plans as needed to support safe and meaningful recovery within the home care setting.

Qualifications
  • Current and active Physical Therapist license in the State of Texas
  • Strong knowledge of physical therapy and rehabilitation techniques across applicable care settings
  • Experience delivering high-quality patient care, preferably within home care or home health environments
  • Solid understanding of evidence-based therapeutic practices
  • Ability to communicate effectively with patients, caregivers, and healthcare professionals
  • Strong organizational and time-management skills with the ability to manage multiple patients independently
  • Proficiency with electronic documentation systems preferred
  • Demonstrated empathy, professionalism, and commitment to supporting patient recovery and independence
