What You Will Do:Conduct thorough assessments within the Connect team triage service, managing urgent cases efficiently.
Develop personalised support plans to promote independence and safety for patients.

Undertake home visits to assess needs, deliver follow‑ups, and provide hands‑on support.
Empower clients with practical tools and strategies to enhance daily living and independence.
