37.5 hours per week (team members work 11.5 hour shifts across a 7‑day week) – Site:
St Barnabas Hospital, Saltash – Salary: £38,682 – £46,580 per annum – Closing date: 20/01/2026 – Job ‑26‑015.
This opportunity is open to internal candidates on secondment. Discuss with your current line manager before applying. An experienced and self‑motivated nurse will join the Acute Care at Home team at St Barnabas Hospital to deliver high‑quality, evidence‑based care to acutely ill patients in the community. The aim is to prevent hospital admission or facilitate rapid discharge.
Main Duties of the Job- Deliver high‑quality evidence‑based care for acutely ill patients within their homes.
- Coordinate with referring hospitals or GP when arranging admission to the scheme.
- Assess, develop, implement and evaluate patient care plans.
- Work a flexible rota shift system covering 0800 to 2030 hours, 7 days a week.
- Cover any area within North and East Cornwall during duty shift.
- Act as lone worker most of the time, but deputise in the absence of Team Leader and support team management, performance and development of staff.

Person SpecificationEducation and Qualifications – Essential
- Registered Nurse (RGN) 1st level registration.
- Diploma‑level specialist training, experience, short courses or equivalent.
Desirable
- Prescribing qualification.
- Guidance Score Prescribing qualification 2.
- Teaching qualification, e.g. City & Guilds, HEA Mentorship module (or working towards).
- First‑degree diploma or working towards one.
Experience – Essential
- Working without direct supervision as part of a team, supported by experience and education.
- Substantial post‑registration experience in an Acute or Community team.
- Experience with a wide range of medical conditions.
- Evidence of autonomous practice.
Desirable
- Evidence of good medical grounding within an acute setting.
- Experience of audit.
