The main duties of the post is to care for individuals with complex health care needs in their own homes within the community.

Are you someone who has an interest working with individuals with complex health needs who wants an exciting new challenge?
Vibrant suitably experienced Health Care Support Workers are required to care for an individual, living in the Torfaen Area.
HCSW's should have knowledge and experience of working with patients with complex health needs. We welcome HCSW's to apply who would be willing to learn and undertake a comprehensive induction. This role will include a rotation of shifts to meet the needs of the service and ensure skill competency is developed and maintained.
Successful candidates will be expected to enable the individual to live a full and productive life by supporting with both physical and psychosocial needs; this includes using the young gentleman's vehicle in the community to assist her in attending appointments and social activities, so a
full UK Licence is essential
. This unique opportunity allows HCSW's to develop a relationship with the young gentleman to understand their individual health and psychological needs and provide high, quality nursing care.

Person Specification Experience- NVQ/QCF Level 2 in Health and Social Care OR equivalent level of demonstrable experience
- A commitment to undertaking continuing personal development and training
- Experience of working in a healthcare environment
- Able to work hours flexibly
- The ability to drive patient vehicles and the ability to travel between sites in a timely manner
