Clinical Case Manager – Veterans Affairs Supportive Housing (JR 5413)

PATH is seeking a Clinical Case Manager to provide case management and supportive services to veterans participating in the HUD VASH Program at the Valley Vista PSH in San Diego, CA.

Location: San Diego, CA 92108

Salary: $33,090 – $40,190 DOE

Employment Type: Full Time, Non‑Exempt

Eligibility: Must have U.S. work authorization, be CPR/First Aid trained, fully COVID‑19 vaccinated, and complete required pre‑employment screenings.

Responsibilities
  • Provide mobile and field‑based case management services in independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and linkages to VA partners and community providers.
  • Deliver strengths‑based case management and service coordination to help veterans obtain and maintain stable employment and housing.
  • Collaborate with clients and VASH/VA staff to develop a Housing Stability Plan (HSP) addressing short‑ and long‑term goals.
  • Assess HSP progress and adjust treatment plans as needed.
  • Offer supportive services focused on enhancing clients’ problem‑solving, coping skills, and care coordination.
  • Utilize evidence‑based practices such as Intensive Case Management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First.
What You Bring
  • Knowledge of advanced, evidence‑based interventions:
    Critical Time Intervention, Harm Reduction, Crisis Intervention, Motivational Interviewing.
  • Proficiency with computers and technology:
    Microsoft Office, database systems.
  • Respectful, non‑judgmental approach with individuals presenting urgent, complex needs.
  • Professionalism, mature judgment, and high motivation.
  • Flexibility and adaptability in a fast‑paced environment.
Preferred Qualifications
  • Current California Board of Behavioral Sciences licensure or registration (e.g., ACSW, LCSW).
Minimum Qualifications
  • Master’s degree in Social Service or Clinical field (MSW, MFT, Clinical Counselor, Psychology).
  • One year of experience in healthcare, case management, homelessness services, or working with vulnerable populations.
Mandatory Requirements
  • Flexible work hours, including evenings and weekends.
  • Employment eligibility verification.
  • CPR/First Aid training available.
  • Full COVID‑19 vaccination proof.
  • Pre‑employment screenings:
    Tuberculosis test, background screening, drug test.
Driving
  • Reliable transportation, valid California driver’s license, and personal vehicle insurance.
  • Eligible for PATH’s insurance coverage.

California‑Based Work Expectations

All regular and customary work must be performed within California. PATH does not offer fully remote positions; hybrid schedules are limited to approved off‑site locations (not home). Work schedules are determined by role responsibilities and may be adjusted.
