Lincoln / Omaha Metro (Home-Based)

Employment Type:

Full-Time (PRN optional, if applicable)

Position Summary

The Home Care Physical Therapist provides high-quality, patient-centered physical therapy services to individuals in their homes. This role focuses on restoring function, improving mobility, relieving pain, and promoting independence while supporting patients in the comfort of their own environment.

Key Responsibilities
  • Evaluate patients in their homes and develop individualized physical therapy treatment plans
  • Provide skilled physical therapy services in accordance with physician orders and plan of care
  • Educate patients and caregivers on exercises, safety, mobility techniques, and injury prevention
  • Monitor patient progress and modify treatment plans as needed
  • Accurately document evaluations, treatments, progress notes, and discharge summaries
  • Collaborate with interdisciplinary home care team members (nursing, OT, speech, social work)
  • Communicate effectively with physicians regarding patient status and progress
  • Maintain compliance with state, federal, and organizational regulations
  • Support patient independence, safety, and overall quality of life
Qualifications
  • Graduate of an accredited Physical Therapy program
  • Active Physical Therapist license in the state of Nebraska
  • Home health or outpatient experience preferred (new grads welcome)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage a flexible schedule
  • Valid driver’s license, reliable transportation, and current auto insurance
  • CPR certification (or willingness to obtain)
