A health solutions company is seeking a Health Survey Specialist to conduct outreach and perform Health Risk Assessments for DSNP members. This role requires at least 2 years of customer service experience and the ability to effectively communicate and support members over the phone. Successful candidates will have strong organizational skills and a commitment to serving others. Flexibility with work schedule to meet business needs is essential, along with proficiency in Microsoft Word.

Full-time position with anticipated 40 hours per week.
