Case Management Professional

Fraser is seeking Case Managers to support individuals on the CADI/BI and DD waivers. The role requires compassion, strong organizational skills, and a commitment to client success. Experience working with individuals with developmental or intellectual disabilities and excellent documentation and case noting skills are essential.

Responsibilities

• Advocate for Fraser clients and help them access resources and services within the community.
• Provide client-centered services for DD and CADI/BI waiver clients.
• Meet with clients 2–3 times per year.
• Maintain accurate and thorough client documentation and case notes.

Caseloads

• DD Waiver & Non-Waiver: average 43 clients (mix of children and adults).
• CADI/BI Waiver: average 35 clients (primarily adults, some children).

Requirements

• Valid Minnesota Driver’s License with acceptable record.
• Effective communication in English (verbal, written).
• Basic mathematical skills, including accurate billing.
• Computer proficiency using Word, Excel and keyboarding skills.
• Excellent organizational skills and attention to detail.

DD Case Manager qualifications

• One of the following:
  Bachelor’s degree in Social Work, Special Education, Psychology, Nursing, Human Services, or related field; or one year of related experience (QDDP qualified); or completion of at least one course focused on developmental disabilities.
• If degree is Social Work, Social Work licensure is required.

CADI/BI Case Manager qualifications

• One of the following:
  Bachelor’s degree in Social Work, Psychology, Sociology, or closely related field (Social Work licensure required if degree is Social Work); or bachelor’s degree in any field plus one year of experience as a social worker, case manager, or care coordinator; or two or more years of experience in service coordination or case management.

Location, Schedule & Pay

• Hybrid/Remote based at Fraser's Bloomington offices; serves clients in Ramsey County. Full-time, salary exempt, Monday–Friday during standard business hours.
• Pay Range: $52,000 – $55,000.
• Hiring Bonus: $1,500.
