Clinical Care Partner
As a Clinical Care Partner, you will provide high-quality clinical care coordination, intake and health liaison services across aged care programs. This role focuses on empowering clients to manage their health and well-being, promoting self-management, and ensuring culturally sensitive, customer‑centred care. Working collaboratively with clients, clinical teams, and operations staff, you’ll deliver best practice care in line with funding and service agreements.

You bring compassion, clinical expertise, and a commitment to empowering clients to live their best lives. Along with a current AHPRA registration as a Registered Nurse, you’ll have:
- Tertiary qualification in a health‑related discipline.
- Experience in aged care or community health, with strong understanding of clinical and operational risk management.
- Excellent communication and interpersonal skills, with ability to build strong relationships within multidisciplinary teams.
- Proven ability to manage caseloads independently, meet deadlines, and demonstrate strong organisational and analytical skills.
- Strong computer skills and ability to manage documentation effectively.
- Leadership skills with a strong focus on team building, communication and change management.
- Ability to thrive in a fast‑paced environment and solve problems effectively.
- Deliver high‑quality clinical care coordination services within the funding and resources available, including assessment, care planning, referrals, and health education.
- Liaise with healthcare providers and stakeholders to ensure coordinated and effective care for customers.
- Monitor and review customer outcomes, providing timely follow‑up and feedback to relevant teams.
- Maintain accurate and confidential records in line with organisational and legislative standards.
- Collaborate with clinical and operational teams to ensure quality outcomes.
- Provide clinical advice and oversight for complex cases, ensuring best practice care.
- Contribute to continuous improvement initiatives and compliance with aged care standards and governance frameworks.
- Provide education and guidance to clients and families, promoting choice, autonomy, and wellness.
