General Statement Of Responsibilities

The Clinician Case Manager is responsible for direct services with children, adolescents and their families including but not limited to evaluation/assessment; home and community based services; outreach; case and care management; individual and family therapies; crisis intervention; record keeping; collaboration with other professionals and team members and related community services (including consultation and/or community and/or school placements). He/she may be assigned to carry a pager and respond to all calls for assistance either by telephone or in person (as clinically indicated).

  • Provide comprehensive evaluations, treatment plans, and treatment of children and adolescents and families, including evaluation/assessment; home and community based services; outreach; case and care management; individual, group and family therapies, as well as crisis intervention.
  • Implement treatment plans that may include psychotherapy and clinically required support services including but not limited to translation, transportation (only as per agency policy), advocacy and linkage to other needed services.
  • Provide community based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to agency offices, client homes, schools, emergency rooms, shelters, police stations, etc.
  • Carry a pager and be available to provide after‑hour emergency mobile (in‑home, community, emergency room, school‑based, other) services including both phone and face‑to‑face interventions, if assigned based on agency and/or client needs.
  • Maintain contact and provide follow‑up with families requiring this service, including daily home visits and home based therapies.
  • Maintain confidential and any other required program documents.
  • Maintain a level of productivity as determined by the Executive Director and assigned by the program supervisor.
  • Work collaboratively as a member of a team with other professionals and concerned parties involved in cases.
  • Assist in the organization, marketing, planning and implementation of the program.
  • Actively participate in trainings and staff meetings.
  • Serve on agency and/or community committees.
  • Actively participate in supervision and report directly to the supervisor.
  • Supervise assigned staff and/or student interns.
  • Other duties as assigned by the Supervisor.
Schedule Requirements

This position requires availability to work one weekday evening until 8:00 PM
. Two 8:00 p.m. evening shifts per week are strongly preferred
.

Qualifications
  • Must have the ability to maintain the utmost confidentiality of both agency and client matters.
  • Must understand and be able to display the clinical knowledge and expertise necessary to perform crisis intervention.
  • Must be able to display knowledge and expertise in clinical theory and practice as it relates to the populations served by Wellmore, Inc.
  • Must be capable of working as part of a team and effectively communicate with both families and professionals (in writing and verbally).
  • Working knowledge of the greater Waterbury area is a must.
  • Bi‑lingual (Spanish‑English) may be required.
Education & Experience

A Master’s Degree in marriage and family therapy, social work, or related human services field with at least a one‑year clinical internship working with children and/or adolescents is required. A Connecticut Associates license is minimum requirement (LPC‑A, MFT‑A, LMSW). A Connecticut LMFT, LPC, LCSW or Doctorate Psychologist preferred. Two years post‑graduate clinical experience working with children, adolescents, families or providing crisis intervention is preferred.

Experience in group and family therapies is also preferred.

Licensure/Certification /Registration

Valid driver’s license, reliable transportation and valid auto insurance required; CPR and first‑aid.
