Case Manager

Job Title: Remote Case Manager Hourly Pay: $18 - $24/hour

We are seeking a compassionate Case Manager to support individuals and families facing personal, healthcare, or social challenges. • Conduct client assessments to determine needs and create tailored case management plans
• Collaborate with healthcare providers, social services, and community organizations to coordinate care
• Monitor client progress and adjust care plans as needed to achieve goals
• Advocate for clients, ensuring they have access to necessary services like healthcare, housing, and financial assistance

Maintain confidential, accurate client records and documentation

• Facilitate communication between clients, their families, and service providers
• Provide crisis intervention and immediate support during critical situations

• Bachelor’s degree in Social Work, Psychology, or a related field (MSW preferred)

2+ years of experience in case management or social services

• Strong knowledge of social services, healthcare, and community resources

Excellent organizational, communication, and problem-solving skills

Ability to handle a varied caseload independently

• Strong interpersonal skills and experience working with diverse populations
• Valid state licensure or certification as required
