The Optum family of businesses is seeking a Licensed Social Worker to join our team in El Paso, TX. As a member of the Optum Health Enterprise Clinical Services team, you'll be an integral part of our vision to make healthcare better for everyone.
The Licensed Social Worker serves as a key member of the interdisciplinary healthcare team, managing non-HCBS waiver member care to improve clinical outcomes, financial performance, and member satisfaction. This role oversees the member's case within the scope of licensure, provides guidance to non-RN clinicians, and develops, monitors, and adjusts individualized care plans to ensure optimal health across the care continuum.
This position follows a hybrid work from home (WFH) model, with field visits as needed. The schedule is Monday - Friday, 8:00 AM MT - 5:00 PM MT, no evenings, weekends or holidays.
Must reside and hold an unrestricted license in the state of Texas.

Primary Responsibilities:
- Conduct comprehensive assessments (telephonic or in-person) to identify physical, behavioral, and social needs
- Develop, implement, and monitor individualized care plans in collaboration with members, families, and providers
- Coordinate care for members with chronic conditions, disabilities, or high-risk complications
- Facilitate referrals to social services, community resources, and support programs
- Educate members and families on care options and available services
- Serve as the primary point of contact for care coordination and advocate for member needs
- Ensure timely documentation of assessments, care plans, and service delivery
- Participate in interdisciplinary team meetings and support safe transitions of care

Required Qualifications:
- Master's degree in social work (MSW) and current Texas licensure as a Licensed Social Worker
- 2+ years of experience in community health, case management, or clinical care settings
- Experience with STAR + PLUS Texas Medicaid managed care program
- Proficiency with MS Word, Excel, and Outlook
- Bilingual (Spanish)
- Dedicated home office space and access to install secure high-speed internet in home (Cable Broadband, DSL, or Fiber)
- Ability to travel within assigned region; valid driver's license and reliable transportation
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
