** This Opportunity:
** The Intensive In-Home Associate III is responsible for providing intensive family preservation in order to improve individual and family strengths, stabilize the home environment, promote positive family interaction, and prevent the use of out-of-home placement for up to twelve individuals/families.#
** What You'll Do:*
* • Coordinate and oversee the initial and ongoing assessment activities, assist the Team Leader in identifying strengths and utilizing those strengths in order to help individuals and families achieve their stated goals/dreams.
• Convene the Child and Family Team for person-centered planning.
• Complete the initial development and ongoing revision of the Person-Centered Plan and ensuring its implementation.
• Consult with identified medical (e.g primary care and psychiatric) and non-medical (e.g. DSS, school, DJJDP) providers, engage community and natural supports, and include their input in the person-centered planning process.
• Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations. Assist individuals and families in accessing community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health, developmental disabilities and substance abuse services
• Provide and coordinate behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
• Monitor and document the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan.
• Participate in the person-centered planning process.
• Assist with implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person-Centered Plan.
• Provide psychoeducation as indicated in the Person-Centered Plan.
• Assist the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person-Centered Plan.
• Assist with crisis interventions. Provide 24/7/365 on-call access to families served by the intensive in-home team and be a first responder to families in cases of emergencies.
