Job Profile Summary
This role focuses on safeguarding and promoting the welfare of patients and canprovideservices if addition, this role focuses on performing the following Social Work duties:
Provides social workassistanceto patients and families relating to illness, disease,hospitalization and life events in all areas of service as a member of multi-disciplinary team and in liaison with community resources.
Responsibilities also include identification, assessment, and coordination of cases and anticipation of needs related to illness, hospitalization, finances, education, and home/community service.
A professional individual contributor role that may direct the work of otherlower level professionals or manage processes and programs.
The majority oftime is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typicallyacquiredthrough advanced education.
A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience.
Typicallyresponsible for: managing projects / processes, working independently with limited supervision,coaching and reviewing the work oflower level professionals, resolving difficult and sometimes complex problems.

Job Description
Minimum Qualifications:
1.Master's degree in Social Workor PHD from an accredited program.
2.Licensed Independent Clinical Social Worker (LICSW).
3.Five (5) years of experience inoutpatient psychotherapy with adult population.
4.Experienced in brief psychotherapy model, CBT, DBT, and treatment of dual diagnosis consumers.
Preferred Qualifications:
1.Experience with group psychotherapy.
Duties and Responsibilities:Theduties and responsibilities listedbeloware intended to describe the general nature of workandare not intended to be an all-inclusive list.
Other dutiesand responsibilities may be assigned.
1.Provideinitial diagnostic assessments using DSM-V.
2.Develop initial andsubsequenttreatment plans, written in observable, measurable terms.
3.Provide individual, couple/family and group treatment modalities within a short-term treatment model.
4.Provide documentation of treatmentutilizing
SOAP format, accepted LMH and CCSforms which andwhich meets LMH, CCS, and regulatory standards.
5.Maintainscurrent therapy notes, treatment plans, and other written documentation and correspondence according to agency policy and withinappropriatetimeframes.
6.Identifies, using format and guidelines,high riskclients and review these cases monthly with Supervisor and Psychiatrist.
7.Responds to needs of clients and the clinic including responding to urgent care requests and crisis intervention.
8.Available to take on new cases as needed.
9.Responsible for theutilizationreview process as needed per insurance requirements.
10.Providestothesupervisor,monthly statistics as requested.
11.Participates in staff clinical meetings and collaborates with treatment team.
12.Participates in monthly clinical consultation.
13.Participates in LMH Community Benefits Program as requested by the supervisor.
14.Participates in monthly individual supervision
15.Participates in monthly chart reviews as requested by the supervisor.
16.Provides
24 hr.back up coverage as assigned.
17.Participates in on call schedule.
18.Provide back up to the Supervisor and other clinicians during his/her absence including case assignment,monitorof high-risk cases and intake functions, as staffing may require.
19.Participate in special projects as may be requested.
20.Maintains CEUsappropriate to position and which meets licensing standards.
Physical Requirements:
1.Frequent sitting, occasional standing&walking, and lifting of 5-10 lbs.
2.Requires manual dexterity using fine hand manipulation tooperatecomputer keyboard.
3.Requires ability to see computer screen and reports.
Skills&Abilities:
1.Strong mental health generalist skills in working with adult population: youngadult through geriatric.
