The HART service delivers several care & support offers under its registration as a domiciliary care service, including reablement, homecare support, and short‑term crisis support. The role involves providing overnight triage and response, delivering short‑term support, undertaking risk assessments and first visits, and managing emergency situations.
- Provide an overnight (10:30 pm – 7:30 am) triage and response service.
- Deliver short‑term support to people with disabilities, frailty or recovery needs.
- Undertake risk assessments and first visits to individuals who use the service.
- Respond to emergencies, coordinate with doctors or ambulance services, and record incidents.
- Act as the main contact point for the service between 10:30 pm and 7:30 am.
- Provide direct service‑user support, including personal care, moving and handling, and medication administration.
- Complete and maintain accurate records for risk assessments, medicine administration, catheter care, and other delegated tasks.
- Maintain high standards of administrative procedures and documentation to evidence delivery of the service.
- Work shifts between 10 pm and 7 am, including alternate weekends.
- Eligible for DBS enhanced check under the Rehabilitation of Offenders Act 1974.
- Ability and willingness to travel and work from various locations across the County.
- NVQ Level 2 or equivalent qualification in a relevant field, or equivalent experience working with adults in a social care setting.
- Experience working with other agencies or in a multi‑agency environment.
- Knowledge of working with people from minority cultures or groups; commitment to equal opportunities and anti‑discrimination practices.
- Ability to assist with moving and handling of service‑users.
- Strong written and oral communication, negotiation and persuasion skills.
- Ability to prioritise across competing demands and meet deadlines.
- Understanding of the Council’s Adult Social Care strategies, relevant regulations, legislation and service frameworks.
- Ability to work flexible hours on a rota basis between 10:30 pm and 7:30 am, 365 days a year, including weekends, evenings and Bank Holidays.
- Must have access to a vehicle at all times.
