What to Expect (Job Responsibilities)
  • Identify potential admissions and process new resident/client requests
  • Coordinate the resident/client move-in process, including handling admission paperwork and insurance verifications
  • Perform clinical evaluations on resident/client charts to determine appropriate medical services
  • Collaborate with discharge planners, family members, and the interdisciplinary team
  • Ensure compliance with laws, regulations, and internal policies regarding admissions
What is Required (Qualifications)
  • Must hold an unencumbered license as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Occupational Therapist (OT), Physical Therapist (PT), or Licensed Social Worker (LSW)
  • Bachelor’s degree from an accredited school and/or program preferred
  • Prior work experience in a long-term care facility preferred
  • Valid driver's license may be required due to travel necessities
  • Basic Life Support (BLS) certification required
How to Stand Out (Preferred Qualifications)
  • Experience in home-based services as a Certified Occupational Therapy Assistant (COTA) or Physical Therapist Assistant (PTA)
  • Strong interpersonal and listening skills
  • Ability to handle confidential and sensitive information
  • Experience with Customer Referral Management (CRM) and electronic medical records (EMR)
  • Proven ability to coordinate multiple responsibilities effectively
