Ensign Services is seeking an experienced PRN Remote Therapy Documentation Auditor to support therapy documentation review across multiple skilled nursing facilities within our KEYSTONE Strong market. This role is ideal for a clinically strong therapist who enjoys chart review, compliance, and supporting teams behind the scenes—while maintaining flexible, remote work.
Who We’re Looking For:- Licensed therapy professionals with a strong understanding of SNF documentation standards and compliance:
- OT / COTA
- PT / PTA
- SLP
- Conduct remote audits of therapy documentation across multiple facilities
- Review documentation for accuracy, compliance, and clinical appropriateness
- Identify trends, risks, and opportunities for improvement
- Provide clear, constructive feedback to therapy leadership
- Support facility teams in maintaining regulatory readiness and documentation excellence
- Collaborate with market and facility therapy leadership as needed
- Audit therapy evaluations, daily notes, progress notes, and discharge summaries
- Ensure compliance with Medicare guidelines, payer requirements, and Ensign standards
- Monitor documentation consistency across disciplines and facilities
Assist with focused audits, education support, and documentation improvement initiatives - Maintain confidentiality and professionalism in all reviews
- Current, active therapy license (OT, COTA, PT, PTA, or SLP)
- SNF experience required
- Strong knowledge of Medicare documentation requirements
- Experience with EMR systems (Net Health, PCC, or similar preferred)
- Excellent attention to detail and written communication skills
- Ability to work independently in a remote environment
- Prior auditing, DOR, ADOR, or leadership experience preferred but not required
- Flexible PRN schedule
