Job specification:
Full-time (1.0 FTE). This is an hourly and non-exempt position and is eligible for overtime pay over 40 hours per week.
Schedule:
Monday-Friday, 8:00 AM – 5:00 PM
Hourly Rate: $20.00 per hour
Summary:
Care Ring - A fast-growing and highly respected community organization seeks a passionate individual for the role of Access Coordinator for the Physicians Reach Out (PRO) program. The Access Coordinator supports the Care Ring mission of empowering individuals with limited resources to establish and maintain good health by connecting eligible clients with access to comprehensive, affordable medical care through enrollment in Physicians Reach Out (PRO) and coordination of donated medical services that requires regular on‑site work along with remote work flexibility.
ResponsibilitiesPRO Eligibility Intake - Enrollment & Renewal 50%
- Client Interaction:
Conduct eligibility screening and explain program details and policies. - Information Gathering & Verification:
Collect applications, personal histories, financial documents, and income to accurately determine program eligibility. - Processes applications:
Creates and maintains accurate client files; enters and updates client information, as appropriate - Enrollment:
Assigns clients to participating PRO network provider, communicating enrollment information to both clients and assigned providers. Accurately documents all client-related communication in Epic client record
- Process Referrals:
Receive, review, and process incoming and outgoing referrals for patients to specialists or other services. - Patient Communication:
Inform patients about appointments, instructions, and potential issues; answer calls and address concerns. - Provide Eligibility:
Provide eligibility to PRO participating providers - Scheduling:
Schedule appointments, tests, or procedures, coordinating times and locations with patients and providers. - Documentation:
Maintain accurate, detailed records in the EMR, including patient demographics, insurance info, and referral status. - Provider
Collaboration:
Work with physicians and other staff to assess patient needs and determine appropriate referrals. - Resource Connection:
Refer patients to community resources.
- The Covid-19 vaccination is required as a condition of employment
- Education:
High School Diploma or GED, required - Bilingual (Spanish) required – written and verbal
- Medical field experience, 1yr minimum required
- Knowledge of barriers to health care for the low-income and/or uninsured population
- Certified Application Counselor or successfully obtain certification within 90 days of hire
- Proficiency in Microsoft Office, including Word, Excel, PowerPoint and Adobe PDF
- Knowledge of healthcare systems (EPIC), medical terminology, and available resources
- Strong oral and written communication skills
- Ability to effectively collaborate with team members and clients virtually and in-person settings
