Nexus-PATH is located across North Dakota and offers treatment foster care, mental health services and outpatient clinical services.

We have a full-time Youth & Family Skills Coach role open on our new Therapeutic Host Homes team. This role will be working directly with youth & families in the Fargo, ND community.

Therapeutic Host Homes are family-style living arrangements for children ages birth – 18 with intellectual or developmental disabilities (/DD). A child lives with a caring provider in their community surrounded by a circle of support, provided by Nexus Family Healing, instead of living in a group home or institution. This is a voluntary program designed to help children return to their biological families when possible while continuing to receive help and mentorship from their Therapeutic Host Home provider.

Position Summary

As part of a multidisciplinary team, the Youth and Family Skills Coach will provide skills services in the treatment of children, adults, and/or families as prescribed in the client’s developed treatment plan as directed and supported by the Clinical Supervisor.

Schedule/Pay/Location
  • On-site (community based) in Fargo, ND
  • Some flexibility in scheduling based upon client needs
  • Monday-Friday work schedule

Primary Responsibilities
  • Assists clients in using age-appropriate self-management skills, including communication, self-regulation, self-help skills, play, social, and relationship skills.
  • Provides de-escalation and therapeutic supports as needed with clients.
  • Notifies and collaborates with Clinical Supervisor in case of crises and emergencies.
  • Completes clinical documentation in compliance with program expectations; demonstrates strong written and oral communication skills to generate and support program documentation expectations and standards.
  • Attends and participates in clinical supervision and team case consultation meetings in accordance with program expectations.
  • Completes education and training experiences in accordance with program expectations.
  • Ensures minimum hours of direct services as assigned.
  • Maintains compliance with all agency, state, and regulatory standards and remain compliant with all HIPAA regulations.
  • Assists in implementing responses to youth behavior and other skills deficits according to the developed treatment plan.
Required

Education and Experience
  • Minimum of a high school diploma or GED
  • Bachelor’s Degree in behavioral science field, Social Work, or Psychology
  • Must have completed 2,000 hours of supervised clinical experience delivering clinical services to treat mental health and emotional disturbances with a trauma focus
  • Some experience working with at-risk youth and families
  • Ability to pass all necessary background screening requirements per DHS rules and regulations
  • Proficiency in Microsoft Office Suite and data entry skills required
  • Educated on and compliant with HIPAA regulations
  • Valid driver’s license required
  • Must meet state regulating agency and Nexus Home Office driving requirements
Preferred

Education and Experience
  • Bachelor's degree or above in one of the behavioral sciences or related fields from an accredited college or university
  • Knowledge of family-based philosophy and practice
  • Experience in related community-based work, working with families, youth, and trauma exposed children and adolescents
