• Continuum‑Based Care (Clinical Excellence): Deliver high‑quality care across home, senior living, and community settings, supported by a multidisciplinary team in palliative, psych, and neurology.
• Dementia Leadership: Build deep expertise in dementia‑informed care and caregiver support as part of our expanding geri‑psych and cognitive health initiative.

• Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
• Delivering dementia‑informed, relationship-based care across the continuum of care.
• Conducting comprehensive assessments and developing tailored care plans for older adults.
• Managing chronic conditions and performing care with skill and empathy for patients and their families.
• Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
• Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
• Maintaining consistency in documentation, follow-through, and patient communication.

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1‑2 years of clinical experience (geriatrics or primary care preferred).

• Valid driver’s license and reliable transportation.

Pay Range for Posted Region

$140,000 - $200,000 USD
