Overview
The Exercise Physiologist plays a key role in supporting the health, wellbeing, and cardiac fitness of our patients. In this position, you will assess individual patient needs and design safe, effective, and personalised exercise programmes for people with a range of heart conditions.
Responsibilities- Develop and implement safe, individualised exercise programmes for patients with cardiac conditions.
- Provide home-based exercise prescriptions for patients choosing home rehabilitation and monitor their progress.
- Set appropriate exercise-based goals with patients and facilitate return to work or daily activities.
- Teach cardiovascular and strength training exercises individually and as part of group sessions.
- Deliver warm-up and cool-down group sessions and monitor patients during exercise and education sessions.
- Monitor and track patient progress and adjust programmes accordingly.
- Provide education on safe exercise and the benefits of regular exercise for cardiac health.
- Provide comprehensive exercise plans and onward referrals on discharge.
- Maintain equipment and a clean and safe environment.
- Provide telephone support for individuals doing home based exercise programmes.
- Liaise regularly with the clinical specialist team regarding treatment progression and ongoing care of patients.
- Maintain patient records accurately and legibly in accordance with Trust guidelines.
- Keep up to date with guidelines and best practice within cardiac rehabilitation.
- Experience in prescribing and delivering exercise programmes, preferably within a cardiac rehabilitation setting.
- Strong communication skills and a genuine passion for helping others achieve their fitness and wellness goals.
- Knowledge of exercise science, physiology, and anatomy.
- Ability to work autonomously and as part of a multidisciplinary cardiac rehabilitation team.
