Indiranagar, Bangalore
Work Timings: 9:00 AM - 6:00 PM or 10:00 AM - 7:00 PM
Compensation: 5.5 LPA (Fixed) + ESOPs
Work Days:
Monday - Friday (In-office) and Saturday (Work from Home)

Summary:
This role focuses on delivering patient-centered care management, chronic disease support, and seamless care transitions across India. As a Care Manager, you sit at the intersection of clinical care and digital operations - supporting members throughout their healthcare journey by planning, coordinating, and monitoring care. Working closely with doctors, internal teams, and partner hospitals, the role ensures smooth care delivery, regular follow-ups, improved patient outcomes, and a consistently high-quality member experience, while also helping reduce avoidable readmissions.
Key Responsibilities as a Care Manager:
- Assess member health needs and goals to understand clinical risks and care requirements
- Create and manage personalized care plans covering consultations, diagnostics, and preventive care
- Coordinate appointments and services across doctors, hospitals, and internal teams to ensure smooth care delivery
- Conduct regular follow-ups to track progress, satisfaction, and evolving health needs
- Provide basic health education and guidance to support adherence and better outcomes
- Act as the primary clinical point of contact, resolving member queries and care issues
- Maintain accurate documentation and track outcomes to ensure quality and effectiveness
Qualifications & Skills et
- Bachelor of Homoeopathic Medicine & Surgery (BHMS) OR Bachelor of Ayurvedic Medicine and Surgery (BAMS)
- 1 to 3 years in Patient care, care coordination, or clinical roles preferred
- Strong clinical understanding
- Good communication and coordination skills
- Organized and detail-oriented
- Comfortable using EMR / healthcare software.
