Position: Stroke Association Support Coordinator | Home-based and Waveney

This is a fixed-term contract until 30 September 2024. Our services are project-funded, we currently have funding for this service until 30 September 2024.

We are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to be part of the team delivering our Stroke Recovery Service based in Norfolk and Waveney. As a result of an increase in local funding to support stroke services across Norfolk and Waveney, we are currently looking to recruit five new support coordinators to help us deliver our strategy and make a real difference.

Reporting to the Service Delivery Coach in the East of England, you will have the ability to support Stroke Survivors to identify their goals. Using strong assessment skills and a person-centered approach, enabling them to maximise their recovery and improve communication.

Key responsibilities will include :
  • Supporting new stroke survivors and their carers from hospital discharge into the community.
  • Providing personalised information, advice and support.
  • Enabling stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
  • Working with other health and social care professionals across the stroke pathway to ensure high-quality support.
You will have experience in :
  • Experience / background in a caring profession, ideally supporting people with disabilities.
  • Excellent IT skills and an ability to maintain accurate records.
  • An affinity with the values of the Stroke Association.
  • A flexible approach and an ability to effectively manage a caseload.

This role requires frequent travel across a large geographical locality to visit people at home and in community settings across the Norfolk and Waverley area. Candidates must be able to demonstrate how they can meet this requirement of the role.

To fulfill the role, you must be a resident of the UK and have the right to work in the UK.
