This is a full-time hybrid role for an Advanced Clinical Practitioner (ACP) based in Crewe, Cheshire. The role involves assessing, diagnosing, treating, and managing patients' care, working alongside multidisciplinary teams. The ACP will provide high-quality, patient-centered care, contribute to the development of clinical pathways, and ensure adherence to clinical guidelines. The role includes a combination of on-site responsibilities and flexibility to work from home, as required.

• Strong clinical skills in patient assessment, diagnosis, care planning, and treatment
• Proficiency in managing patient records and ensuring compliance with healthcare regulations
• Experience with multidisciplinary collaboration and the ability to effectively communicate with patients, families, and healthcare teams
• Knowledge of medical protocols, clinical guidelines, and healthcare procedures
• Proficiency in critical thinking, problem-solving, and decision-making under pressure
• Registered healthcare professional with relevant qualifications (e.g., nursing, paramedicine, or advanced clinical practice)
• Ability to work independently and adapt to a hybrid working environment
• Experience in primary or urgent care is a plus
