Overview

A fantastic opportunity has arisen within our Enablement Assessment Team as we seek a Team Manager to provide strong leadership and effective management to a diverse group of professionals. The team supports vulnerable adults as they transition from our Enablement Service, out-of-bed bases, and short-stay placements. This role will also play a key part in driving forward the development of the Transfer of Care Hub (TOCH).

The Enablement Assessment Team operates Monday to Friday, with the team contributing to an out-of-hours rota. The team supports discharges across a range of bed bases, completing Care Act Assessments for people with ongoing care and support needs following a period of home-based enablement, as well as assessments for individuals who have stayed in a short stay placement in a residential setting.

The team also undertakes community reviews before transferring people to our locality teams.

As Team Manager, you will provide oversight and support to staff across all bases and work collaboratively with health partners within the Transfer of Care Hub, located near Rotherham Hospital at the Woodside Building. The successful candidate will lead a team of social workers and vocationally qualified staff delivering services to adults with learning disabilities, carers, older people, people with physical disabilities, mental ill health, and/or sensory impairments.

Responsibilities
  • Coordinate caseloads and allocations, lead and contribute to project work, and ensure work is reviewed, monitored, recorded, and evaluated to maintain high standards of service delivery.
  • Provide oversight and responsibility for the performance of the team; work flexibly to meet the needs of the team and service, including planning annual leave and training.
  • Support staff across all bases and collaborate with health partners within the Transfer of Care Hub.
  • Lead a team delivering services to adults with learning disabilities, carers, older people, people with physical disabilities, mental ill health, and/or sensory impairments.
  • Ensure services are delivered in accordance with legal frameworks governing Adult Social Care and in line with the Rotherham Adult Social Care Pathway.
  • Promote strengths-based practice and focus on outcomes for people in their community.
  • Contribute to safeguarding, equality, diversity, and inclusion initiatives as part of the role.
Qualifications and Requirements
  • Experience leading and managing a multidisciplinary team within an adult social care context.
  • Strong leadership, decision-making, and partnership-working skills.
  • Ability to manage performance, supervise staff, and drive continuous improvement in service delivery.
  • Commitment to the principles of strengths-based practice and person-centred approaches.
