Job Title: Community Wellbeing Social Worker (Adult Social Care)
Contract: Permanent
Location: Hereford City, Ross on Wye and Leominster (please feel free to specify your preference)
Hours: 37 hours
Salary: £36,363 to £40,777 + £3,000 welcome payment (please see guidance)

• Is a registered Social Worker with Social Work England and holds a qualifying degree.
• Has a deep understanding of current legislation and its real-world impact.
• Possesses the drive to potentially pursue qualifications in Practice Education, Best Interest Assessor, and/or Approved Mental Health Professional

• Collaborates effectively with external agencies to build strong partnerships.
• Holds a full, clean driving license and is ready to explore our vibrant county.

• We are looking for an advocate with: A Social Work England registration and a relevant qualification.
• Up-to-date knowledge of legislation and guidance.
• A track record of effective, person-centred assessment and care management.
• The drive to develop into a Best Interest Assessor or Approved Mental Health Professional.

• Take on a variety of adult social work, including Care Act Assessments and person-centered support planning, as well as Mental Capacity Assessments and Section 42 Enquiries

• Apply experience with diverse adult populations, including physical and learning disabilities, mental health needs, and older adults.
• Use a strength-based approach to empower individuals and champion independence.
• Engage in crucial CHC and s117 work to ensure seamless support.
• thrive in a supportive culture that offers regular supervision and opportunities for professional growth.

• Transform lives with comprehensive Care Act Assessments and bespoke support plans.
• Navigate complex situations with expertise in Section 42 Enquiries, Mental Capacity Assessments, and more.
• Collaborate fearlessly with partner agencies to ensure holistic care.
• Never stop growing with regular supervision and funded opportunities to specialize.

• A salary range of £36,363 to £40,777 + £3,000 welcome payment (please see guidance).
• Enrolment in the Local Government Pension Scheme (LGPS).
