Stroke Assistant Practitioner

We are excited to offer an opportunity for an enthusiastic and highly motivated Stroke Assistant Practitioner to join our continually evolving Integrated Community Stroke Service.

Our Stroke Early Supported Discharge (ESD) Team, established in 2009, has recently undergone an exciting period of growth and development. Since April 2023, the service has expanded to increase access to high-quality, specialist, community-based stroke rehabilitation across the localities we serve. This high-profile service supports patients across Mansfield, Ashfield, Newark and Sherwood, providing rehabilitation following an acute stroke and helping people rebuild their independence closer to home.

The service plays a crucial role in supporting patients during the first six months following discharge from hospital, ensuring a smooth, timely transition from hospital to home. Our focus is on maximising recovery, function, and independence, underpinned by a strong ethos of patient‑centred care, compassion, and consistently high clinical standards.

We are looking for someone with a genuine interest in stroke care and rehabilitation, who is motivated by making a real difference to patients' lives. This role offers the opportunity to work as part of a supportive multidisciplinary team, develop your skills, and contribute to the ongoing growth and innovation of the service.

Qualifications
  • NVQ 3 or equivalent qualification in health related subject
  • Knowledge and skills acquired through training or experience in Physiotherapy, Occupational therapy, Speech and Language Therapy in working with rehabilitation of adults
  • Validated Courses in relevant subjects
Experience
  • Experience working with older or disabled people in a care or rehabilitation setting in health or social care
  • Working as a member of a multidisciplinary team.
  • NHS Experience
  • Experience of working with stroke patients
Physical Requirements
  • Physically able to carry out role
  • Well‑developed handling and observational skills
Contractual Requirements
  • Working across localities as role demands
  • Able to cover 8am
    -6pm including weekends and bank holidays to meet the needs of the service
  • Car Owner / Driver - Full Driving Licence
