Job Summary
Provide leadership in developing, implementing and evaluating evidence-based dementia and caregiver support programmes. Supervise the team’s clinical practice, lead outreach efforts and engage stakeholders to ensure person-centric service delivery. This role supports the rollout of non-pharmacological services for persons with dementia, covering preventive to intervention work, in collaboration with partners and volunteers.
Key Responsibilities Areas- Conduct comprehensive assessments for clients and caregivers using the Bio-Psycho-Social-Spiritual (BPSS) model to develop individualised care plans.
- Provide case management, counselling, and care coordination for individuals with mental health conditions, including dementia.
- Deliver Care Connect’s programmes, including:
- Reminiscence Connect (RC): Thematic groupwork using reminiscence to support cognition in seniors with mild cognitive impairment or early-stage dementia.
- Cognitive Stimulation Programme (CSP): Evidence-based group activities for persons with mild to moderate dementia.
- Eldersitter Programme: Home-based befriending and therapeutic engagement to maintain cognitive function in persons with dementia.
- Superheroes Programme: Group-based support and empowerment for caregivers of persons with dementia.
- Plan and implement community outreach, groupwork, and preventive mental health initiatives.
- Work closely with community partners to support clients with complex psychosocial and healthcare needs.
- Conduct public talks and workshops to raise awareness of mental health and dementia.
- Recruit, train and support volunteers serving as befrienders to clients with mental health needs.
- Perform other duties as assigned by the Reporting Officer, Head, Community Mental Health and the Executive Director.
- 3 to 5 years of relevant experience in mental health and dementia care, with demonstrated competencies in case management, group work, programme development, and community engagement.
- Prior supervisory experience will be an added advantage.
- Graduate Diploma, Bachelor's or Master's degree in Social Work, or Nursing or Relevant Allied Health Qualifications.
- Willingness to travel and conduct home visits within the designated service boundary.
- Strong knowledge of case management practices for individuals with mental illnesses and dementia.
- Familiarity with current trends and developments in the health and social care sectors.
- Effective team player with excellent interpersonal and communication skills.
- Bilingual abilities and knowledge of dialects will be an added advantage.
