The Neighbourhood Alliance in Sussex are looking for dynamic, collaborative individuals to join us as Neighbourhood Integrators, supporting the development of Integrated Community Teams (ICTs) across Sussex.

This is an exciting opportunity to play a key role in delivering the NHS Neighbourhood Health Plan by helping partners work together to provide more joined-up, community-based care closer to home.

Working at the heart of a local ICT, you will bring together organisations, teams, data and insight to strengthen collaboration, improve outcomes and reduce health inequalities.

You will work across organisational boundaries, influence system change, and help build more connected, responsive services for local communities.

There are 15 Integrated Community Teams (ICTs) across Sussex, with an ambition to appoint a dedicated Neighbourhood Integrator to each.

This recruitment represents the first phase of appointments, with further opportunities expected as the programme develops.

Main duties of the job

As a Neighbourhood Integrator, you will:

  • Enable the leadership and coordination of a local ICT
  • Bring partners together to enable effective, joined-up working
  • Help develop and deliver integrated, person-centred care
  • Support multidisciplinary teams (MDTs) to work effectively
  • Use data and insight to inform local priorities and action
  • Identify and help address gaps in services and outcomes
  • Strengthen links with community and voluntary organisations
  • Reduce duplication and streamline ways of working

We are looking for people who:

  • Have experience working across health, care, local government or community settings
  • Are confident working across organisational boundaries
  • Can build strong relationships and influence without formal authority
  • Are organised and able to coordinate complex programmes of work
  • Are passionate about improving outcomes and reducing inequalities
  • Are comfortable working in dynamic and evolving environments
About us

This role is hosted by the Primary Care Provider Collaborative delivery arm, one of the core partners within the Sussex Neighbourhood Alliance.

The Neighbourhood Alliance brings together NHS organisations, local authorities, social care and voluntary sector partners to deliver integrated neighbourhood health services across Sussex.

Job responsibilities

Accountable to: ICT Leadership Group and Sussex Neighbourhood Alliance (Line management delivered through PCPC as hosting organisation)

Proposed salary:

Contract: 12-month fixed term

Base: Community locations, partner sites and home working

1. Context

The NHS Neighbourhood Health Plan sets out a shift toward delivering more care at home or closer to home, creating integrated, community-based services that improve access, experience and outcomes for people of all ages that support people holistically, recognising the interaction between physical health, mental health and social determinants of health.

It aims to build healthier, more independent communities by embedding joined up working between the NHS, local government, social care and voluntary partners as the norm rather than the exception. In Sussex we have formed the Neighbourhood Alliance to enable the delivery of this plan.

The Sussex Neighbourhood Alliance (NA) forms part of the wider Sussex Provider Collaborative (SPC), which brings together Sussex NHS provider organisations to jointly deliver large-scale transformation programmes across the system. The Neighbourhood Alliance is one of the Collaboratives core strategic alliances.
