Adult Social Care Practitioner

We have a full‑time (37 hours) Adult Social Care Practitioner position available within our Mental Health Team in Gloucestershire. The role requires travel across the county and is undertaken as part of a supportive team led by a Head of Service, Deputy Social Care Managers, Social Work Leads, a Social Care Lead, Social Workers, Adult Social Care Practitioners and an administration team.

The Mental Health Service at Gloucestershire County Council supports adults who have social care needs arising from a primary, non‑organic mental health condition. Our service works across community and inpatient settings, providing high‑quality, person‑centred support that empowers people to live as independently and safely as possible.

• undertake assessments and support plans for people who have a primary health condition;
• work with families, carers and other professionals, agencies and organisations across Gloucestershire;
• promote independence, enable individuals to remain in their own homes and reduce the need for hospital admission or long‑term care;
• apply the Care Act in practice and comply with the Immigration Act 2016 requirement to demonstrate fluent English in spoken aspects of the role.

• relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience;
• training in Health and Social Care assessments;
• experience managing a caseload including people with a range of physical disabilities, medical, mental‑health difficulties and learning difficulties;
• good interpersonal and verbal/written communication skills;
• proficient IT skills.
